is the administrator of personal data whose sources and the purposes and scope of processing are object of the present Declaration.
Personal data processed by PTHO derive from the following sources
a) Membership declaration
b) Data update forms
c) oral, written or electronic requests for supplementation or modification of the data
d) correspondence carried out by traditional mail, courier or electronic mail
The data processed by PTHO may include the following information: name, surname, professional number, date of birth, academic degree, address of residence, place of work, specializations held, telephone number, e-mail address, date of admission to PTHO and membership statute.
a) Admission to the PTHO of a person expressing such a wish through a membership declaration;
b) implementation of rights and obligations of the Society’s members
c) implementation of statutory objectives and tasks set out in the statute of the Polish Society for Hyperthermic Oncology – current statute is available at www.ptho.pl/statut.
d) implementation of legitimate interests of the controller (eg monitoring of electronic mail, monitoring of the IT system, establishing, investigating or defending claims in court, administrative or other extrajudicial proceedings),
e) In other cases, personal data will be processed only on the basis of prior consent granted to the extent and purpose specified therein.
a) PTHO statutory authorities (General Meeting of Members, Board, Audit Committee, Peer Court).

c) entities that process personal data on behalf of PTHO on the basis of a personal data processing agreement concluded with PTHO (so-called processing entities), the purpose of which is to remain within the scope specified in item 4 above.
a) At present, PTHO does not transfer or plan to transfer your data outside of the European Economic Area. However, if such a situation takes place in the future, the processing will be carried out using standard contractual clauses approved by the European Commission, in order to ensure an adequate level of data protection required by law.

Personal data of PTHO members are processed throughout their membership of the Society.
After the membership is finished, the data is not processed, but only stored in the form of archival records.
Other personal data, processed on the basis of written consent, may be processed until the objection is raised regarding their processing for until the Administrator determines its obsolescence.
Every person whose personal data is processed by PTHO is entitled to:
a) the right to access her data and receive a copy thereof;
b) the right to rectify (amend) her data;
c) the right to delete data or to limit the processing of data;
d) the right to object to data processing;
e) the right to data transfer;
f) the right to lodge a complaint to the supervisory body (the President of the Office for the
Protection of Personal Data).
In the event of opposition to the processing of data on the basis of a legitimate interest, PTHO will cease to process the data concerned, unless there are legitimate grounds, the execution of which prevents the cessation of processing or will be necessary to possibly establish, assert or defend claims.
At any time, you have the right to object to the processing of data for marketing purposes.
